Mental is a miniseries about the main SpongeBob cast struggling with various mental disorders as adults. Specifically, SpongeBob struggles with multiple personality disorder (MPD), Patrick struggles with binge eating disorder (BED), Sandy has Tourettes syndrome, and Squidward identifies as a walrus.

The gang all discuss their first therapy sessions after deciding to get help together. SpongeBob is put on antidepressants to help try to control his personalities, however the medication fails and emerges a new, extra-destructive personality; Squidward confronts Sandy with a large portion of cash for her to attempt to perform surgery to make him physically a walrus. Patrick asks SpongeBob to stay at his house, only to help him not binge before an upcoming high school reunion. Meanwhile, Squilvia struggles with Squidward's desire to become a walrus and think it is simply a fetish, leading to some odd and insulting bedroom play. (TV-14-DLS) (BBFC 15)

SpongeBob's new personality sends him out into reckless, extremely dangerous behavior, while another personality forces him to do anything he can to make it up to those he's hurt, which leads to an odd adventure in the night life scene; Sandy questions the legality of the procedure Squidward has asked her to perform on him; Patrick tries out various fad diets and finds loopholes in every single one of them, and finds a pro-binging/weight-gain site along the way. After one of SpongeBob's personalities emerges and says nasty things about Patrick in public, both struggle with thoughts of suicide as they both fall into a deep depression. After receiving a pile of bills and seeing her own financial struggles from the costs of her own treatment, Sandy decides to go through with Squidward's surgery. Patrick is found dead in his home in quite a gory scene as the consequence of his over-the-top binge. In a fit of utter rage and despair, SpongeBob's most destructive personality poisons the stevia in Sandy's tea, leading to medical and legal consequences.
